ABSTRACT

Objective To study the effect of preemptive analgesina of morphine injected to subarachnoid space on postoperative epidural analgesia with morphine. Methods Sixty ASA Ⅰ-Ⅱ patients were randomly divided into two groups. All patinents received combination spinal-epidural anesthesia(CSEA). In experimenta group (n=30), 0 5% bupivacaine 2ml and morphine 0 5mg were injected into the subarachnoid space of the patients for CSEA, and morphine 2 8mg was used for postoperative epidural analgesia. In control group (n=30),0 5% bupivacaine 2ml was injected into the subarachnoid space of the patients for CSEA, and morphine 3 3mg was used for postoperative epidural analgesia. The numerical rating score (NRS) and Ramsay sedation scores were performed after operation. The postoperative analgesic duration and frequency of side effect were recorded. Rusult NRS was significantly less and postoperative analgesic duration was obviously longer in experimental group compared with control group. Ramsay sedation scores and the frequency of side effect significantly increased in the experimental group compared with the control group. But the shaking frequency in experimental group was significantly less than that in the control group. Conclusions Preemtive analgesia of morphine injected to subarachnoid space could improve postoperative epidural analgesia with morphine and increase analgesia time. But it had more side effects.

ABSTRACT

Objective To explore the treatment of traumatically amputated penis and the causes of success and failure in the varieties of procedures used to heal the wound and restore function. Methods Two patients with traumatically amputated penis were treated by operation. Results In one case the operation was successful, and in the other case it was unsuccessful. Conclusion There were numerous variables contributed to the success or failure of penile replantation. The most important factors were contamination and exsanguine time of the amputated portion of the penis. The actual surgical procedures and the appropriate pre-operative and post-operative treatments were crucial to the outcome as well.

ABSTRACT

Objective To determine the degree of C 4 changes at precede or coincide with changes in systemic lupus erythematosus (SLE) activity by 5 global activity indices(PGA, M-SLEDAI, M-LAI, SLAM, and M-BILAG), and to evaluate the correlation between changes in C 4 levels and SLE activity in individual organ systems.Methods 53 lupus patients were observed monthly for 1 year in a longitudinal study. Lupus disease activity rate and complement levels were measured at each visit. Disease activity rates were calculated for subgroups defined by previous or concurrent changes in C 4 levels. Logistic regression models were used to determine the significance of the correlation between recent changes in complement levels and disease activity, and between changes in C 4 levels and SLE activity increased in specific organ systems.Results Lupus disease activity occurred at 12%,25%,13% and 12% respectively on PGA,M-SLEDAI,M-LAI,SLAM and M-BILAG. Disease activity by the M-LAI were more frequent when there was a concurrent decrease in C 4. Higher disease activity rates by the SLAM were correlated with previous increases in C 4. Decreases in C 4 were correlated with a concurrent increase in renal disease activity,or related to a concurrent decrease in the hematocrit levels and platelet count.Conclusions Reducing serum in C 4 levels were not consistently correlated with SLE disease activity, decrease in C 4 was correlated with a concurrent increases in renal and hematologic SLE activity.

ABSTRACT

Objective To culture astrocytes of human optic nerve and establish the cell lines for further study of healing process after optic nerve trauma. [WT5”HZ]Methods [WT5”BZ]Astrocytes of infantile optic nerve were cultured by tissue inoculation or tissue digestion with 0.25% trypsin and 0.06% EDTA. The second and fourth passage cells were stained with HE and anti GFAP, S 100 protein, vimentin, and CD34 antibodies. [WT5”HZ]Results The trypsinized astrocytes of infantile optic nerve reached confluence in 7 days. The cultured cells were in polygonal shape with processes and the cytoplasm was abundant. These cells were positive in GFAP, S 100 protein and vimentin staining, and negative in CD34 staining. Conclusions Astrocytes of human optic nerve can be successfully cultured by trypsinization rather than tissue inoculation.

ABSTRACT

Objective To test the hypothesis that attachment of synovial cell to &?2-microglobulin modified with advanced glycation end products (ACE-?2m) would affect cell adhesion, spreading and proliferation.Methods Normal human synovial cells (type B cells) were isolated and plated in culture dishes coated with AGE-?2m or with normal extracellular matrix proteins (EMP). Adhesion was analyzed by counting the isotope-labelled cells. Spreading was tested using a light microscope and proliferation determined by 3H-TdR incorporation and counting the number of cells. Results Synovial cells adhered less effectively to AGE-?2m, ?2m and AGE-collagen than to the normal EMP (collagen and fibronectin). Cells interacting with AGE-?2m, ?2m or AGE-collagen also demonstrated less extensive spreading throughout the examined time intervals (60-120 minutes after plating), and decreased 3H-TdR incorporation and cell numbers after 72 hours of plating when compared to cells interacting with normal EMP. Conchusion AGE-?2m in amyloid may alter synovial cell behavior in situ in ways which cods contribute to the development of dialysis-related amyloidodsis(DRA).
